No student shall be admitted for the first time to any public kindergarten or elementary school in a school division unless the student shall furnish, prior to admission a comprehensive physical examination of a scope prescribed by the State Health Commissioner performed within the twelve months prior to the date that the student first enters such public kindergarten or elementary school, or records establishing that the student furnished a report upon prior admission to another school or school division and providing the information contained in the report.

No student shall be admitted by a school unless at the time of admission the student or his/her parent submits documentary proof of immunization on the State’s School Entrance Health Form to the admitting official of the school or unless the student is exempted from immunization for medical or religious reasons, the student is a homeless child or youth or the student is in foster care. Rising 6th Graders Need T-DAP Vaccine Before New School Year Starts
All students entering sixth grade must present proof that they have received the Tdap vaccine for tetanus, diphtheria, and pertussis before starting school this year.Virginia rules changed in 2014. Now, all sixth-grade students are included. There is no exemption for those under 11 years of age.
Parents should check with their doctor to be sure their children have all required vaccines.Proof of immunization or documented exemption must be presented before students begin sixth-grade classes for the 2015-16 school year.
Shots may be obtained from a doctor, military clinic, or the Prince William Health District. Take a copy of proof of the Tdap vaccine to your child’s middle school as soon as possible between now and the start of the new school year and keep a copy for your files.
